Brian,
From anywhere in your app, click on the Users tab,
on the right top of this screen, select Manage Roles
select the role that you want to limit (creating a record)
now locate the table of your drop-down selections.
uncheck the "Add" column for that table.
You, as an admin, will always see the add new record option in the dropdown unless you test your app as the role in question to validate that role can no longer make record additions. This is located in your username dropdown for modifying preferences and billing.

Ok I went in and changed the "Notes" table to Modify = "None", the result of that is when I do adjust the role to verify the role cannot make a record in my tables is I can't open the form anymore. I change it back to "All Records" and I can see the form again.
Are my settings in the screengrab I attached incorrect?

Are you at least an app admin or realm admin?
I did not mention updating your table "Modify" column to "none" - that is incorrect.
It also sounds like you are operating under the same role as the role you want to limit the addition of records

Hi Chuck, I am both an app admin and a realm admin.
When I am making these changes I am an app admin. The issues arise when I change the role to "Warehouse MGR Admin" the role I am trying to limit the access.
To view the image right click on it and open it in a new tab. Works for me.
Something to note, the form opens in the new form style not the old one. Not sure why as I didn't tell it to do that.

Your set up looks correct, but can you also look at the user interface tab for that role and make sure that the option to hide the Add record button is selected. I suppose that there might be a bug on the new form, which is showing the add category option when the user doesn't have permissions to add a category.
Then the only other option I can suggest if that does not fix the problem is to make a form for categories in the old format, and make sure that it's being used for all roles. And then do your test again. If it works properly on the old form, then it must be a bug with new forms that needs to be reported.
