CSV Handler Pipeline not Importing records

I am attempting to fetch a CSV from a username / pw authenticated website. Once retrieved, I want to import the csv contents into a QB table.  I've never done this before but I think I am super close...

It looks like you've just copy/pasted the sample headers into your header field. Do you NEED these headers?
