It might be easier to ask Support to look at this with you since they can look at the app directly and the answer will be specific to your app.
In my experience, troubleshooting permissions is best done with a buddy. That way, someone always has admin access and you can tweak the other person's permissions until you find the elusive detail that needs to change.

I found that it has nothing to do with the role. It's the popup form. Any one know why the pop up form would have this issue?
