Exact Forms Error Message

Question

Hello,
Using the 64 bit Word doc template, attempting to either open an Exact Form from Quickbase or Save As to Quickbase, I receive the error message below. I am following this guide.
If I purposefully enter an incorrect password, the resulting error message is a password error, so my credentials are correct. I have application creation permissions (am admin).
Any thoughts on troubleshooting steps?

Thank you. I have tried an Application Token and a User Token in that field. Same error.

Can you try without antivirus or firewall?I think it may be a network output rule.

Same result with firewalls and AV turned off.

I did try in word 2007 NOT WORK. (same error)I did try in word Office 16&nbsp;&nbsp; GOOD work. I think it is possible your word version.
