File Attachment

Something happened to cause my ability to attach files to my projects.  This has been working for over 2 years.  Is something on the backend of the QB changes cause this or is it local to my environment?

The PS Doc below is where the File Attachment field resides:

      The normal process is to click an "Add PSP" button which allows a new PSP record to be created. Along with other fields, the PSP Doc is where we upload a copy of the SOW - usually a PDF.  The "Choose" button does not seem to work anymore.  When hitting the Choose button, the new window which allows you to pick your file locally or on a network drive does not appear. The Choose Button does not work anymore.
