Hiding URL information at top of PDF

Hello,

Is there a way to remove the portion that is underlined in the attached screenshot? I would like to prevent that from showing up in forms when I PDF it and send it to a customer.

    No problem. This is actually a setting on the browser on your own PC. 

    The location of the settings depends on which browser you are using but typically it is somewhere in the print menu when you go to print and you set your headers and footers.

    Basically if you can find that they will be defaulted to show the URL the page and you want to not do that. They may also default to show the current date and time and you probably want to get rid of that too.

    Once you make that set and change it will stick for the rest of your life on that particular browser and computer.
