How to add the text field parallel to multiple fields

If you only need this format for when the user is "viewing" the record (as opposed to "editing" or "adding"), then you could create a formula - rich text field that consolidates all of the information in whatever format you are able to create using HTML and inline CSS.

    I'd like to format so it displays in both view and edit mode. Do you have suggestion?

    I don't.  In edit, you are restricted to the minimal layout options unless you create a custom form.  I'm like Champion and generally stick with the native options.
