I'm looking to create a pop-up form (checkboxes, fields of entry)...

I'm looking to create a pop-up form (checkboxes, fields of entry) upon a change in field (in this case an change in Ops Step actioned by way of the Kanban report).  How would I create such a popup, and how/where would that information be captured?
