Last Week's Time as a Personal User Report Subscription

I have this exact subscription setup to go out on the 25th day of the month that sends a report filtered by current user, regarding their last months submissions. I've confirmed with a couple individuals, who have records on this report regularly, that they are not getting the subscription report. I only learned this after I didn't receive it myself.
I have the blank report box unchecked so that users who don't have data on the report don't get an email. Not sure if this is messing with the report sending at all. My only thought is that since some users wouldn't have records on this report, no one is getting it as a result even if they have records on it.
Details: - Report is filtered by a Username QB = Current User - Username QB is not a column on the report. This shouldn't be an issue to my knowledge, but included it in case it is for some reason I'm unaware of. - Notification Details:
