Logged Field with only the User Name and Date?

I typically use a Multiple Choice field to log a user selection. So, perhaps I will have them select 'Approved' and it will save: User Name: Date Approved.

Is there a way to simply have the user name and date logged? Essentially, I want the same as the user clicking a checkbox and logging that.

My initial idea is to just make a formula field and strip everything out I dont want. Is that the best method?
