Logo in exact forms

Hi Everyone,

I have some exact forms that i added in my Quick Bass table.  In the Word template that QB gave me I added the logo but when I open it in the table to print the logo isn't there.

    Take a look over the instructions for including images here and make sure that you've followed each step exactly.
