Looking for formula checkbox to check if a record was created in the first 5 days of the current month

This should do the trick:

var date created = ToDate([Date Created]);

var date firstday = Date(Year(Today()),Month(Today()), 1 );

var date fifthday = Date(Year(Today()),Month(Today()), 5 );

$created >= $firstday and $created <= $fifthday
