It sounds like you might be having some issues with authenticating your sign in session to Quick Base when making use of Exact Forms. I haven't heard of any error messaging regarding SIM cards with Exact Forms before. If this issue is persisting I would recommend putting in a support case so that you and a rep can take a look at your set up together on a screen share to see what might be going on. There is an alternative 64 bit version of Exact Forms we have access to if that turns out to be the issue.
