New Form not opening from Report

Hi - We built a new form using the new form building and created a report that is setup to view the new form.  We have been able to edit/view the records from this report (showing the new form).  But today, the record won't open.  It seems to be in a loop trying to open the URL.  We have not changed anything in the table/report/form.  Thanks for any help!
