QR Code to Open form and Add new Record

Question

I have an app with a URL to a form to generate a new record when submitted. I would like to generate a QR code that when I use iPhone camera to open the QR code the user can fill it out and submit. When I create the QR code and use the iPhone camera it opens a different form in the app. Is there a setting in QB that controls the form that opens?
