In your original post you gave a screenshot which is pretty unreadable and then you copy paste or just a portion of your code. So the only code I tried to fix is the portion of the code that you posted b y copy paste.
If you're saying that the code has a syntax error, then you need to show me the entire code for the field because if you just actually took what I suggested that was only a small portion of your formula.

The code is massive and all of it worked prior to adding the bit I copy pasted to you. but attached is a notepad with the code as i think it would be overwhelming to paste here.The part i pasted to you is the only thing i changed in the code. let me know if that works. Thank you!!

That is a nice sized formula. I suggest breaking up your development into pieces and then stringing them together. It helps me read and debug. Also since you have large formulas, I would recommend downloading Visual Studio Code and the Quickbase extension. It is so easy to make a simple mistake and need to roll back to the prior formula that works. You cannot do that unless you put your work into a source control system. I use GitHub. It is cheap and easy.

I can confirm that this works as a stand alone. If you have another error in the larger field, debug unrelated to the PHQ Follow Up is needed.
