Sorry, I actually was meaning to show a screen shot or a portion of the form on a real record showing the blank choice being offered to the User.

Well blank is not a choice. Blank is the current value of that field. Because the field is required as indicated by the red star, that means you are not allowed to select blank.
If you use the form properties to display the choices as radio buttons, you will probably prefer the look on the form.
