URL Help

I want to add a link to a form to the CA State Sales Tax website but all I'm getting in the form is a blank text box.  I got it to work on a testing app I have but not on my real app.
Here's what I have (everything is exactly the same on my testing app)

I can't figure out what's different and why its not working.  Anyone know what is missing?

    A URL field is only meant to be typed into by the end user. If you want it to display something you should create a Formula - URL field and add the full URL to that, I believe it must be enclosed in quotes.

    There is a little bug that also happens with it displaying the URL instead. Don't click the option "Don't show 'http://' when showing the URL" and you should be able to have it named like in your screenshot.
