Forms/Reports - owner listed on screen

Please see attached.  Is there a way to remove the owner of a form or report from showing on the screen?  I am the App owner and App manager, however the user that originally created this is listed at the bottom.  Due to external users seeing this, I need to remove it or change it.  Screenshot attached.

    np,

    Just "Customize this Form" and then edit the form properties tab and de-select

    Show built-in fields

    But if you still need other fields like they create it and date modified, then you will have to add those fields onto the form manually
