Duplicate fields being generated within a record, initiated by another field

Hello,

When someone selects from the options from one field, I want an existing field to duplicate on the basis of the selections and the field to be slightly renamed, for example:

Field: Which bodies will receive different amendment documents?
Responses/options: REC; MHRA
Fields: REC Amended Documents (version and date)
MHRA Amended Documents (version and date)

The original field name is 'Amended Documents (version and date)', and will be modified on the a basis of the selections chosen in the preceeding question.

Will I need a formula to do this and if so which one?
