Customize landing page for "ask the user" report

Question

I am creating an ask the user report and would like to add some instructions or clarifications to the mail report page (where the requested fields show for input).&nbsp; Is this possible?&nbsp; If so, can anyone point me to a help article or site to help me configure it?

I don't think that you can add descriptions there other than by having very wordy field names.&nbsp;
