Customize page icon and column filter disappear

There was a filter under each column in the report but not sure when/why it keeps disappearing and how to activate it back? Also., I noticed Customize page icon is not visible sometimes. user...

The button that shows/hides the filters is this:  



You'll only see the 'Customize this Form' Button when you are viewing a form (as opposed to a report).  If you are viewing a report it becomes this icon:
Also your role needs to have permission to customize the form or report to see this option.
