How can i get the all questions in the

how can i get the all questions data in one request that raised by users.
eg  
1. Request table
2 question table 

Request table :   user will raise the request for the review then reviewer see all the questions for each  request and  enter the comment for all the questions.

Question Table 

	Record ID	Questions
	1	A
	2	B

Request Table

Request ID   populate all question (eg Both  question in all the request )

Eg Request id(1) has A question and B question from the question table.
Request id(2) has A question and B question from the question table
.Request id(3) has A question and B question from the question table.
