How to build table heading values as related table values in dynamically?

I Have a Master Table is Doctors


When Edit Doctors Table Record it's look like below screen



In the above screen In milestone Section i added milestone name like OT3,OT1,etc and these data stored in Milestones Table.

My Requirement is these milestone names OT3,OT1,... as table header columns like below screen

And these Table can be editable and we can fill data and save to values their respective columns like OT3,OT1..
