how to create crosstab report

I need help creating a cross tab report - below is an example of how i need the data to lay out - 

I am just not wrapping my head around how to create the report in quickbase - HELP!!

    Have a "Table" report created with necessary fields and use "sorting & grouping" option (in Customize this report) to display the report in the way you wanted.

    If something have to be totaled (calculated), then try creating a "summary" report with necessary fields and use "sorting & grouping" option

    Yes, that is what I am trying - but it only lets me choose one field to group by - as you can see, I need to group by 3 different fields, and they are "nested" 

    If it is possible to create this in either report option, please provide some more specific instructions?  Because I am NOT getting it on my own
