Noting or logging account when notification is triggered

Within our main app, we have customer accounts and I have a check box  that when checked will send an email to a person with the request to make a change to the account.  I would like to be able to also "auto-note" the account so there is a record or trail.  I am a tad clueless on where to begin to figure this out or perhaps I am going about this all wrong...any help would be appreciated!

    One simple solution would it be to create I text Pheobe and enable logging. That is a field property.

    Then you could have a form rule to say that when your check box is checked which triggers Email, to write certain words into that text field with logging enabled. When logging is enabled it records the date and who did the update.

    That is one simple solution if you just want to make a note of the date that the request was made. There are also a whole range of other solutions depending on your needs.
