Quickbase App Formulas

I have a "Daily Inspections" form that is mainly for use on the Quickbase app for tablets. It has a dropdown list of equipment, and will display and require the Pass/Fail inspection items for the specified equipment when selected. My issue is that I have for each equipment a "Pass All Checks" checkbox, that when checked is supposed to change all items to "Pass". When it is checked, it is usually only passing the first item and leaving the rest blank, and then have to be manually filled in. A second issue is that a comments box at the bottom is supposed to be required upon any item being marked as "Fail", but that is not happening either. It all seems to work on the desktop version however. Any insight? Thank you.
