Your set up looks correct, but can you also look at the user interface tab for that role and make sure that the option to hide the Add record button is selected.  I suppose that there might be a bug on the new form, which is showing the add category option when the user doesn't have permissions to add a category.

Then the only other option I can suggest if that does not fix the problem is to make a form for categories in the old format, and make sure that it's being used for all roles. And then do your test again. If it works properly on the old form, then it must be a bug with new forms that needs to be reported.

    Ok I went in to the User Interface tab for that role and clicked the following.

    Still not working.  I included the image below just to show what it is I am trying to get rid of.  The Create topic link at the bottom of the dropdown.

    Is there any way to convert my form to the old one style?

      There is no conversion tool to convert from new to old or old to new forms. But what I suggest you do is a to make a super mini old style form just with this drop-down field on it and see if it behaves properly or not.

      Ok just a follow up here.  I took Mark's advice and created a form using the old style form creator and upon testing the form in the role I am trying to limit it works a expected.  It seems there may be a bug with the new form setup?  

      Anyway, thanks guys for all your help.

        I would strongly encourage you to put in a bug report. They will be very responsive to bugs on new forms. You can even include a link to our discussions here so they can replicate the problem.
