Reference Guide/Quick Lookup

I have a raw materials table that has a "description", "$/ea", "$/ft", "markup" and "List Price" columns.  What I need to do is get all of the list prices for each individual raw material to its own field so I can use that specific field ID on a form on EF+. What is the easiest way to point the form to a specific field in a list like this? 

Below is the word document it needs to end up on with Field ID's to reference.

    If you are using the Juiced Exact Forms Plus add on, you should be including an embedded subtable in the setup.  It sounds like you are trying to hard code this into the main Word Document as fixed information on a header record.
