Report is not getting refreshed after adding new data

Question

In my report, I've this questions section. I added questions but its not getting added over here. I do see entries in the respective table.

I'm not sure what is missing. Please advise.

The first thing to check if if the child is being linked to the Parent. Try putting the field for either the [Related Parent] on the form set to read only or else a Proxy field for Related Parent.
