Tag a survey note and send an email to external user

The business ask is to be able to "tag" one or more Notes on a survey form and then send that note(s) and its related information (site name, address, note type, etc.) to an email address they would type or select from a predefined list of external site contacts.

Looking for general direction on how to solve for this.
