This part here looks wrong. Did you exactly copy and paste the button code. Does the formula show an error?

I have copied the exact code as listed under the URL field "sign it". The code does not show any kind of errors except the one that pops up when trying to submit the signature

I probably cannot help you because I don't really have the skills to do code pages but can you post the code which is on page number 2. Maybe someone else on this forum understands code pages and what they might be trying to do.
Plan B would be to implement the Juiced Add on for Signatures
