When field is edited, prompt for additional data

Hi

I have an app where measurement data is entered for quality assurance.  Generally speaking once data is entered, it shouldn't be changed, except in rare occasions when a mistype needs to be corrected, or it was determined some other error occured that requires the data to be updated. 

I already created an audit log table and built an automation that creates a record when certain key fields are edited, copying the original data, new data, timestamp and user info. 

I would like to add to this a "Notes" field or somewhere the user can enter why they changed the data.  It's for QA so it's important to know why it changed.

Is there a way to prompt the user to enter notes in a different field when editing a specific field?
