CSV Handler Pipeline not Importing records

I am attempting to fetch a CSV from a username / pw authenticated website. Once retrieved, I want to import the csv contents into a QB table. 
I've never done this before but I think I am super close to getting it - I'm able to retrieve the CSV but when it comes to importing it into my table; nothing shows up. I'm pretty sure it has something to do with the Header setup but my research keeps going around in circles. I think I'm likely doing something pretty basic incorrectly. 

My pipeline setup is screenshotted, any help would be great!

    It looks like you've just copy/pasted the sample headers into your header field. Do you NEED these headers?
