While you were definitely correct about the field value being blank in the relationship, I went ahead and changed this:

Qrew LegendAre you using a report to control the drop down list on the form? Check the blue link on the form for that drop down field. What is the first field on that report? It should be the employee name.
Unfortunately i did not build this application, or any of the functionality. I was hired on recently and am patching holes in the system so take what I have to say with a grain of salt ...
so, i believe that is what is happening. from what I have seen in other tables and app it looks like the original builder has a lot of 'reports' baked in to concatenate information like the name, and then in turn use that concatenated "report" as a lookup field in other places. It mangles things pretty quickly.
i believe this is the screen you are asking to see? the first few fields are just information for users filling out the form; the first actual option on the form is the drop down. Please correct me if I'm wrong and you would like to see a different screen.
