IOL Not Working

Hello!

I am trying to get the IOL technique to function in one of my forms, and I have followed this tutorial:

However, when I load my page I get the error:

It looks like it is trying to load my data?

I have a Rich Text Formula, and the HTML I have is:
"<img qbu=\"module\" src=\"/i/clear2x2.gif\" onload=\"javascript:$.getScript(gReqAppDBID+'?a=dbpage&pagename=module.js&rand='+new Date().getTime());\">"

I am not entirely sure what I've done wrong. It seems like it's simply failing to parse the HTML itself.

Can anyone help me with this? Has anyone run into a similar issue?
