How to create an action from a table that creates a new record in another table that is a unique new record even if the parent table has multiple of the same name?

After dreaming about solutions for this last night, going for a long walk with the dog to clear my head this afternoon, and hours of trial and error - I figured it out! I am by far not a programmer but got very creative with native QB functionality and it worked! Sorry about the long/confusing subject line - it was a confusing puzzle to explain. Persistence pays off and it's only because of this great QB community's support for everyone, even if I didn't get any responses on this post. :)
