Regard summary report

Is there any way to create the summary report link this in QB. I have tried several ways not able to acheive like this. Any help is appreciated.

    Elandirayan,

    I am not sure I understand your question.

    The image you posted is a Summary Report.  Are you trying to put this into a form or on to a dashboard?  Perhaps you want to share the report externally via a hyperlink?

    A more detailed explanation of your use case would be helpful.

    You should be able to make a summary report and group by Activity Type and in the summarize what section , select those fields which are in your columns.
