The expedient solution is to go to the settings for the table and set the correct Record Picker fields under Advanced settings. That will give you three fields to select.
If you need more that three fields or need to control the sort or apply some filters to the list, you create a record typically called Used on my XYZ form and make any report you like, then set the form properties to use that report for the drop down list.
