If a customer might regularly have several concerns and you want to be able to track them all separately or be able to assign them out to different teams or team members to resolve one of the best ways to do this is to create a concerns or issues table related to your table of Customers. This way every time a customer has a concern you can create a record for that issue. Then if you want to be able to quickly enter them you can put a report link on the Customer's table tied that is pulled from the issues table so you can see their previous issues and quickly add new issues. To be able to add them in the lines of the report right on the Customer record form you would just need to go into that report link's form elements and select the 'Editable' checkbox inside the form elements, this will then make it possible to add new concerns right on the report so you can quickly capture several issues.
