If you look at the list of notifications, is the green checkbox enabled? Is the notification active?

Yes. Under the tables Settings/Notifications, subscriptions & reminders it is listed, has a green check in the "Active" column, the user listed under "Owner" is in the admin role with full access to the app and table. Nothing is in red.

If that does not fix the problem then either QuickBase support will have to have a look at it or if you want I don't mind having a look at it. If you want that contact me directly by the email in my signature line.

[Project Reference]
E.g., subject = "Request to approve project: [Project Reference]"
When I remove the field reference, it works.
The field referenced is a formula text field that concatenates several other fields. It also, and this is what I'm sure is causing the issue, contains some HTML. When I click preview in the rule edit form, the subject shows as:
Request to approve project: <div style="text-align: left">COMPANY-TestChanged 1958-06-16 to 1958-06-16 TestChanged</div>
The field reference works in the body of the e‑mail but not the subject line. But the body supports HTML, the subject line obviously does not.
