The GenAddRecordForm opens up an Add Record form, you are confusing it by trying to open two forms

try this

URLRoot() & "db/" &[_DBID_AUDIT_FORMS]
& "?a=API_GenAddRecordForm"
& "&_fid_9=" & URLEncode([Helipad_Name])
&  "&_fid_10=" & URLEncode([FAA ID])
