HELP! Broken relationship possibly - reports empty

I was working in a table with relationship to another table.  I think I might have changed something I shouldn't have...possibly even deleted a relationship that I shouldn't have.  I need some help to...

Whether it is a simple tweak with your existing table-to-table relationship settings or a complete app restore, the Care team will be able to help troubleshoot!
