Are you wanting to have a link to the form from the Home Page?
What is the use case scenario?

Thanks for responding. I wanted to have one home page for our sales reps. I needed to make it really simple so I made a for that has only the information they would need on the road. The form is perfect - I just want that to be their home page. Thanks Lori

It's an unusual request, but you can make a Home Page and drag in the Web Page widget.
Then configure the widget with a copy paste of the URL to the record that you want to display, but add the suffix to it of
&ifv=1
If the page does not render, then you will need to put in a support ticket to ask Quickbase support to "Enable iframes for your Realm"
This all assumes that your goal is to embed a single particular record on a Home Page.

I would add a button to the homepage for the Sales Reps. The Sales reps can click that button and get right to the add form.
To get the right URL for your button, go to the table with the form you want to use. Then click the Blue button in the top right, "New (Record Name)". Once you are in the add record form, copy the URL in the address bar and then add it to your new button on the Sales Reps home page.
