Combine existing field in multi choice field

I have a section in my app for booking appointments. I want to be able to pre-fill the appointment location with either the clients address (which has its own field in the app) or a different address such as our head office etc. Since I'm a newbie, I'm not even sure if it is possible to combine an existing field  with a drop down list? It would also be great if the user could add additional locations if need be.

So what is normal in QuickBase is that there would be a default address loaded on the Customer File which would be looked up in a Relationship to the Appointments table.

Then, what i would probably do is to set up a table of standard override addresses.

Then you would set up that relationships as well. ( 1 standard address has many Booking appointments)

Then the final address used for all report and forms in view mode would be a formula field that would be something like

if([Related override address] >0, [Standard address], [Customer default address]).  You hide the override field in view mode on the form for tidiness..
