Formula's can only be Unique if you use the Record ID# field as part of the formula. For what you're trying to accomplish, it sounds like you would want to have a Formula that calculated the key value and a Text field that would actually be the key field, with a Form Rule that moved the value of the Formula to the Text field when the record is saved.
Certain field types are not allowed to be used in the formula which is being marked as Unique, [Record owner] is not allowed. If you you only want one record per quarter per person, you will need to find another method.

For example, to have the user sect their name off a drop down list.
