Hyperlink in Rich text formula
I am trying to make the following work.

To trouble shoot:
- Remove the new code from the original formula and save it. Does the prior functionality come back? If it does not then you need a deep dive into that field as something other than your addition has changed.
- Create a new field and only put in the logic for PHQ Follow Up. If this works as a stand alone then you have it correct. Your issue is to surgically insert it into the existing field with correctly placed & and commas.

The original huge formula works perfect alone. I did end up figuring it out. I should have been linking the hyperlink from elsewhere in the form rather then trying to add it directly.
