make date time unique along with a text field

I would like to create a formula field that will concatenate a few text fields along with a date & time field.


My fields are;


Appointment Date, Provider Name, Case Manager

Create the formula text field and try the below code.
ToText([Appointment Date]) &"  "& [Provider Name] &"  "& [Case Manager]

Just to add, if your Case Manager field is a User field then, try UserToName formula.
UserToName([Case Manager])
It says "This formula is incompatible with this field being marked unique"

My date field was a Date & Time field. I changed it to just "date" and it works. I made a dedicated time field and included it into the formula. Thanks

Hi Steph,

I would suggest trying something like the formula below without the conversion. If it is a formula text field it should force the values to convert. Then as Gaurav mentioned you may need to adjust to UserToName if that is a user field that would instead show an email, etc. 

[Appointment Date] & " " & [Provider Name] & " " & [Case Manager] 

This should allow you to string together those three fields with a space between each one. You can also switch out each " " for a "-" or ":" if you wanted a character between each part of the entry.
